NWN named to CRN’s 2015 TE250 list

NWN Corporation is proud to announce that they have been named to CRN’s 2015 Tech Elite 250 list again this year!

The CRN’s Tech Elite 250 list includes solution providers who stand out by devoting time to gain certifications in the area of infrastructure.  Gaining knowledge and certifications from partners allow valued workers to gain technical skills and knowledge about their partners as well as their offerings.

NWN has taken advantage of the training and certification opportunities offered by our partners and have gained proficient knowledge of each IT vendor, allowing us to be named to CRN’s 2015 Tech Elite 250 list.

Amerigroup-Curaspan Pilot Findings Result in 12-Month Extension. Increased enrollment in community health programs spurs inclusion of Maryland HIE.

Newton, Mass. – Curaspan, the leading provider of care transition solutions, and Amerigroup have announced a 15.7 percent reduction in readmission rates for high-risk members across five Baltimore hospitals as part of a year-long pilot program.  The goal of this program was to enable payers, hospitals and select community providers to proactively collaborate and enroll high-risk members in community health programs prior to hospital discharge.

“We were able to work with five hospitals, four provider organizations and Amerigroup to significantly impact the care of complex members by bringing about transparency and collaboration with the use of Curaspan’s technology,” stated Rob Fisher, Vice President of Hospital Sales at Curaspan.

Amerigroup utilized Curaspan’s care transition technology and its established network of hospitals in Baltimore to receive electronic notifications when high-risk members presented at any of the five participating hospitals. The notifications provided Amerigroup with opportunities to connect the hospitals with specialty community providers to enhance member care management.  Susan Garcia, Director Health Care Management Services, Amerigroup, explained “Hospital care team members can now collaborate in real-time with our payer care team, which has resulted in greater enrollment in community health programs and reduced readmissions.” Specific results of the pilot include:

“It’s a privilege to work with a like-minded partner,” described Tom Ferry, President & CEO of Curaspan. “Amerigroup and Curaspan were able to dissect a very complex and costly problem into segments that could be addressed by practical and achievable solutions ultimately driving better collaboration between payers, providers and members.”

Encouraged by the pilot’s success, Amerigroup and Curaspan have extended the program for an additional 12 months. This year’s pilot extension will also incorporate data from the Chesapeake Regional Information System for Our Patients (CRISP), a leading health information exchange serving Maryland and the District of Columbia.

Engaging with CRISP will enable Amerigroup to receive alerts and notifications for high-risk members who present to any Maryland or D.C. hospital. This will dramatically expand the program’s coverage and has the potential to significantly increase notifications around member events, leading to more transition interventions and better patient outcomes.   Ferry elaborated, “We look forward to learning more as we extend the pilot for another year, expand it to include CRISP, and continue with incremental innovation.”
